Player Profile Questionnaire

Please take a couple of minutes to answer the following questions to enable me to set up a player profile for you (or update your profile if you already have one).

Please also supply a recent photo to go with your profile.

Make sure that you email your answers from your own student email address so that I know that you've really answered the questions yourself and it's not someone else pissing about giving us duff info!
